Employee Records Payroll Related Consequences
All staff should complete the “Update Form” each quarter.

Licensed staff must keep their license current or your eligibility to work will be revoked.  Penalties and/or consequences may also be assess.

Pay periods are 1-15 and 16-end of month.  Pay days are the 7th and 22th of the month. Timesheet’s are due by end of the 15th and 31st. Warning

Vacation Sick Complaining
2-weeks prior notice required.  Less notice results in loss of points and/or write-up.  Vacation pay benefits are not available to everyone.  Issued at discretion of management. Medical note required otherwise see disciplinary action.  No sick pay is offered. Grounds for termination.  See “General Job Description”.
Officership Breaks Overtime
All officer tasks are privileges. All staff are allowed to a 30 minute break for every eight hour shift.  Notification to all supervisors and co-workers are required prior to taking a break.

A “break” is defined as personal time.  During a break staff member may snack, rest, make personal call(s), check email, etc.

No overtime is allowed for any staff.  All work is to be completed within the operation hours.  No patients are to be scheduled outside of operation hours.

Officership tasks are to be completed within operation hours. It’s the option of the employee to perform officer tasks outside of operation hours.

Social activities are NOT to take place during operation hours.  Should a social activity be planned, it’s the staff members option to perform tasks in or out of the operation hours.

Hearing Impaired/Deaf Patient Rights and Policies

When working with patients with hearing impairments, it is essential to communicate important and significant information regarding their care in a manner in which they can clearly understand.  If the patient is not visually impaired, we should use notes or computer.  If they have a family member that signs, we can request that the family member be present during examinations or re-evaluations.  Physical therapy is inherently different than other medical services in that we do not typically handle life-threatening cases.  Many physician offices also customarily use alternative auxiliary aids such as notes, computer, or family members to communicate necessary information as well. 

Should there be any extenuating circumstances where you feel a special interpreter service be required, you may submit a formal request to the administrator.  The request should include all of the following:  patient’s name, diagnosis, details on any/all specific impairments/deficits, reason for requesting special interpreter service, whether or not they have a family member that signs, why the more common auxiliary aides are not sufficient, number of visits anticipated, treating therapist, and the name of the provider making request.

Team Meetings Team Trainings Eating Together
First 30-minutes every Monday morning.

Second Wednesday of every month from 12-2pm, the staff should gather to discuss statistics and facility performance.

Trainings should take place every other Wednesday from 12-2pm. It is recommended that all staff dine together at least 2-3 times each month to builcamaraderie. Preferably prior to team trainings and meetings.
